I was a very new Doctor Who fan, started with the fourth series, so when I saw this I didn't understand most of it - I'd never even heard the phrase EXTERMINATE! But I had seen enough to like the show, and I liked the character of Donna. So I had enough explanations, because she was new and had to have everything explained to her. Her ending made me very sad and this final made me want to see more of Doctor Who. I have seen every episode of the New series in just 1 week, and now I am a complete addict and a very great fan. After catching up with the previous series I watched Stolen Earth/Journey's End again, and this time everything made sense, and I was impressed how everything came back from all 4 series - even Bad Wolf! I'll admit that not everything is great - the regeneration felt like a cheap way to get a good cliffhanger, and Rose's comeback, although I loved the reunitement at the end of Stolen Earth, seemed somewhat unnecessary- she doesn't contribute anything to the story.
However, The character of the doctor and his life are examined in a more detailed way than ever and that is the main storyline, along with the return of the companions. The battle with the Daleks is almost not important, it's just a background for the rest of it. The ending made much more sense the second time around, because I had seen him say goodbye before, and I knew his loneliness. The final scenes makes this one of the best episodes so far, and I can't wait for the next ones.
