The victim pleads against it but the surgery proceeds while the victim is numbed but awake. It is not quite explicitly shown, but there is a video camera trained on the procedure and an out-of-focus TV monitor can be seen with vague forms of internal tissue and organs. Sound effects can also be heard. : Later in the film, it is found out that the man was not actually castrated
