Newspaper reporter Rachel Keller (Naomi Watts) and her son Aidan (David Dorfman) have moved to a small town, Astoria, Oregon, to get away from the terrifying events that happened to them in Seattle. Rachel now works for the local newspaper, The Astoria Gazette, when she hears on the police scanner that the police are investigating the mysterious death of a teenager. The audiece watches how he had tried to make a nerd-like friend of him watch the infected video, sentencing her to death but saving himself. As she is finally clever enough not to watch the video, he is killed.

Rachel goes to the crime scene and discovers that Samara Morgan (Kelly Stables and Daveigh Chase) is back, starting a new cycle of death by taking her revenge on the people of Astoria. Aidan does not only watch the videotape played, he is taken inside the video. Samara attacks him because Rachel burnt the videotape.

The next day mother and son go to a funfair, where the boy takes pictures of himself with Samara. Also, they are attacked by a herd of deers, or noose. Aidan also begins to lose body temperature, so Rachel takes him to the hospital. Hospital doctors find bruises on Aidan, and Emma Temple (Elizabeth Perkins) believes that Rachel has been abusing him, so she forbid Rachel to visit him anymore. Aidan is in a coma, but he wakes suddenly, and says he wants to go back home with Rachel, never calling her "mum". As Dr Perkins won't allow it, he forces her to give an injection to and killing herself, saying that he can't stop.

To stop what is happening to Aidan, Rachel goes back to the home of Samara Morgan in Seattle to find the truth. Samara had been adopted, so she looks for the biological mother, Evelyn (Sissy Spacek) impriosoned in a mental hospital. Evelyn had gone through a very difficult pregnancy and a hard post-birth depression, and had tried to kill Samara as a baby (Caitlin Mavromates) without success. Evelyn tells Rachel than the only way to save Aidan is by killing him by drowing him, and that he'll tell her so. The male nurse (Victor McCay) tells Rachel that she's not the first mother who has asked Evelyn for help.

Aidan goes back to Rachel home, where Max Rourke (Simon Baker), a journalist who is Rachel's workmate and friend talks to him. Max disappears, leaving behind only his denim jacket. Aidan can only communicating when sleeping now, because if he's awake Samara can hear everything. Samara allegedly never sleeps. Aidan asks her mother to kill him while she's sleeping, until a crack in a plank of wood is heard.

Samara is back: she now lives on Aidan's body. The first changes in Aidan are noticed by Rachel: he never sleeps anymore, he watches TV non-stop and he calls her "mum" all the time. She gives something to put Aidan to sleep and she drowns him. Samara wakes up and leaves Aidan's body, but immediately she appears on the on TV screen. She pulls Rachel into the screen and down into the well where she died.

Rachel climbs up, as the mouth of the well is not completely closes. Samara follows her and grabs her by her ankle. Rachel puses Samara down, who plunges once again into the black freezing water, only to start climbing up immediately. But Rachel succeeds in going out of the well and closing the big stone lid on top. She gets lost in a misty forest, but arrives at the stop where Samara's adopting mother committed suicide. She jumps and wakes up close to Aidan.
