- Errors made by characters (possibly deliberate errors by the filmmakers): Misspelling of the word "Disappearance" in one of the newspaper headlines at the end of the movie
- Continuity: Adam leaves the bathtub and walks away from the tub leaving the shower curtain open. A little later, the shower curtain is closed
- Continuity: Towards the end of the movie, Jessie goes to check out the answering machine, then freaks out and pushes it away. A little later Paul tries to phone her up and we see the answering machine back on the table again.
- Continuity: When Roy's body is discovered, Adam's family pulls up and the car windows are all up. When Paul looks back at Adam in the car, the windows are now down.
- Continuity: When Adam's father steps out of his car on the bridge, Adam's bicycle moves from the middle of the bridge to the side of the bridge, but we don't see anybody move the bicycle.
- Continuity: As Jessie walks to the photo gallery, her coat changes from a non-hooded coat to a hooded coat when she stops to look in the window. As she walks inside, she again has the non-hooded coat. When she walks out and down the street to take the call from Richard, she has the hooded coat again with sunglasses.
- Crew or equipment visible: On the bridge, after looking down at Roy's body, as the camera moves towards Adam in the car, a shadow of the camera is revealed on the car (lower left side of the screen).
- Continuity: At the beginning of the movie, when Paul is mugged by his former student, he drops the present he is carrying and the wrapping becomes loose. When he picks it up, the wrapping is still intact.
- Continuity: When Paul almost hits Adam2 while he's on the bike, the bike was about two feet in front of the car. When Paul is hugging Adam2, the bike's rear wheel is clearly under the car.
- Continuity: When Paul and Dr. Wells get into the fight in the church immediately after the funeral, all the people attending the funeral and the church employees are suddenly gone.
- Errors in geography: When Paul Duncan is driving, a Canadian flag is just about visible in the background.

- Factual errors: SPOILER: Richard Wells first introduces himself as Jessie's teacher in college. However, it's later revealed that he is Zackery Clark's father. Neither Dr. Wells nor Jessie ever bring up Dr. Wells changing his last name and the babysitter's appearance/age didn't change enough to assume that Dr. Wells had changed his name before meeting Jessie.
