Robert Thorn's wife is pregnant and is going into labour. The day is the 6th of June and it is 6.00am. When he gets to the hospital, he is informed that his childs was a stillbirth. A priest then offers him the choice to adopt a baby boy who has no parents, he is reluctant to do so but knowing his wife will be devestated by the news of the death of her real child he agrees to the adoption.

The boy is called Damien and moves to England with his mother and father when his father is appointed US ambassador to England. When Damien is celebrating his fifth birthday party, the family's nanny hangs herself in front of everyone and at the same time, the house is being watched by a rottweiler dog. A new nanny, Mrs.Baylock is sent to replace her but it is clear that she is not as she seems. A priest who knows something sinister about Damien begins stalking Robert Thorn. He confronts him on several occasions, warning him about Damien and how he is the antichristHe also tells Robert Thorn about the fact that his wife is pregnant again and Satan will not allow the child to be born. After their last meeting, the priest dies in a bizzare accident when he is impaled by a church spire.

Sure enough, when Robert gets home he discovers that his wife is pregnant but does not wish to carry on with the pregnancy. Robert insists that it goes on as planned. Later that day, Damien's mother falls over a bannister after being knocked over by Damien, injuring her and thusly killing her unborn child. A photographer contacts Robert and shows him photographs that he had taken of both the priest and his first nanny before they died. On the photographs, there are ghostly images of what would eventually kill them such as the church spire on the priest. He also shows Robert the priest's living quarters. The walls were covered in crosses and bible pages and his autopsy showed that his body was riddled with cancer and he had a birthmark on his left thigh in the form of 3 sixes. Robert wants to find out the truth and so does the photographer after taking a picture of himself with no head

The two of them travel to Italy, where he adopted Damien and finds that the hospital he was born in burned down five years ago along with all the birth records. They go and find the head priest at the hospital at a monestery only to find him im very grave health. He is severly burned and has movement only in his left hand. Using a piece of charcoal, the priest points him in the direction of Damien's true mother. The mother is buried at an old abandoned graveyard. They find two graves which they proceed to open, in the first they find a jackal skeleton which confirms that Damien was born of a jackal and is not human. In the next grave, they find the skeleton of Robert's true son who was murdered at birth. They are then attacked by a pack of Rottweiler dogs and narrowly escape the cementary.

Robert calls his wife who is in hospital and tells her to come to Italy. Although she is thrown out of the hospital window by Mrs.Baylock as she gets up. Robert is devestated by the news and he and the photographer go to Israel to find a certain archaeologist who knows how to stop the antichrist. When he meets the archaeologist, he is shocked to discover that he must stab Damien with the seven daggers of meggido on holy ground in order to kill him. He throws the daggers away and the photographer runs to retrieve them. However a pane of falling glass slices his head off. Robert returns to England with the daggers intent on killing Damien.

When he returns to his house he finds a rottweiler dog waiting for him, he succeeds in trapping it. Robert then takes some scissors and searches Damiens scalp for a birthmark in the form of a series of three sixes. Upon doing so, he discovers the sixes and all doubt about Damiens true identity are diminished. Mrs.Baylock then attacks him and as he takes Damien down the stairs. He fights her briefly before stabbing her with a kitchen knife. He then drags Damien into his car and speeds off to the local church. A police officer sees him speeding and pursues him. Robert drives to the church with several police cars on his tail. He goes into the church and takes Damien kicking and screaming to the alter. He is about to stab him with the daggers, but he is shot by a police officer and dies.

The film ends with Robert Thorn and his wifes military funeral. Damien has been adopted by Roberts brother, who is the president of the United States. Damien looks into the camera and smiles somewhat menacingly as the prophecy of the antichrist is being fullfilled....
